I can confirm some details: This perk has 3 ranks with increasing cost in charisma. In the edurance perks section their is a rad resistance perk that reduces radiation by a percentage each level but with the ghoulish perk radiation heals you.
And Using A Hazmat Suit Or Power Armor Makes It Completely Useless, Because You Absorb Next To No Rads.
This perk synergizes well with the ghoulish and radicool perks, as well as martial artist and a very fast melee weapon such as. Now while truly ghoulish is a step in the right direction, in my opinion completely removing all radiation risk at rank 1 was too. (higher rad environment, higher healing).

Basically radiation heals you, and does not give you radiation damage, as with a feral ghoul when shot with radiation. Description like many i think, i found the ghoulish perk a bit lacklustre to say the least, then i noticed truly ghoulish by jasonmbrady that adds rad removal of 1000 to the ghoulish perk at rank 1, i thought great someone's finally fixed it. The new system has been merged with the skill system of the previous fallout titles, meaning that it is different from them.
Which Means If You Have Rad Resistant Clothing Or The Rad Resistant Perk, Ghoulish Will Be Less Effective.
This is just a question i have pondered since trying out the ghoulish perk. (no healing the red bar) it does heal you based on the rads you are being affected by. Each consecutive rank provides more healing through radiation.
